Patient Registration Form We respect the privacy of your personal health information. To assist us in preparation for your consultation appointment, recent x-ray or information will be helpful. Please give us approval to contact your other health provider by completing the “consent for Release of Personal Health Record” form and return to our clinic via fax or email. Consent for Release of Personal Health Record Reader What to Expect: 1st appointment: ~40 - 60 min. Initial Assessment and Examination during which time Yours or your child’s medical history and (oral) health status are reviewed and accessed Oral or dental care need is diagnosed, available treatment options will be discussed Sleep Dentistry Consultation and arrangement for treatment “asleep”, as appropriate 2nd appointment: TREATMENT appointment Pre-op instruction must be followed for safety reason Medical update and treatment will be rendered while asleep with our clinical care team You or your child will be closely monitored in our clinic until deemed safe to return home Patient must be accompanied home by a responsible adult via taxi or private vehicle (absolutely no public transportation) Please follow our post-op instruction closely and do not plan for return to work or school for another 18-24 hours
